CONSENT TO SERVICES

I voluntarily consent to receive esthetic, holistic, and wellness services provided by Esthetics by Monica, which may include but are not limited to:

  • Facials & advanced skincare treatments

  • Chemical peels (including glycolic, anti-aging, corrective peels)

  • Facial massage, sculpting, face yoga

  • LED therapy

  • Microchanneling or skin resurfacing services

  • Reiki & energy healing

  • Holistic or relaxation-based services

I understand that all services are elective and results vary from person to person. No guarantees have been made regarding outcomes.

MEDICAL & SKIN HEALTH DISCLOSURE

I confirm that I have fully disclosed all relevant medical conditions, allergies, medications (including Accutane history, retinoids, antibiotics), pregnancy or nursing status, skin sensitivities, and mental or emotional health considerations that may affect my treatment.

I understand that failure to disclose accurate information may increase the risk of adverse reactions, and I accept full responsibility for such outcomes.

I agree to inform my provider of any changes to my health prior to each appointment.

ASSUMPTION OF RISK

I understand that all services involve inherent risks, which may include but are not limited to:

  • Redness, irritation, peeling, or sensitivity

  • Temporary discomfort or swelling

  • Breakouts or pigment changes

  • Emotional release, fatigue, or energetic shifts (especially with Reiki or holistic services)

I knowingly and voluntarily assume all risks associated with the services I choose to receive.

CHEMICAL PEEL SPECIFIC CONSENT

I understand that chemical peels work by exfoliating the skin using chemical solutions and may result in visible peeling or sensitivity.

I acknowledge that:

  • Strict adherence to pre- and post-care instructions is required

  • Sun exposure, improper aftercare, or skin picking may increase risks

  • Results and healing time vary

I understand that potential side effects may include redness, flaking, hyperpigmentation or hypopigmentation, and in rare cases scarring or infection.

REIKI & ENERGY HEALING CONSENT

I understand that Reiki is a complementary energy healing modality intended to promote relaxation, balance, and overall well-being.

I acknowledge that:

  • Reiki is not medical or psychological treatment

  • It does not diagnose, treat, or cure illness

  • Emotional or energetic responses may occur

I consent to Reiki sessions fully of my own free will and understand I may stop the session at any time.

NO MEDICAL OR MENTAL HEALTH CLAIMS

I understand that services provided by Esthetics by Monica are not a substitute for medical, dermatological, or mental health care. I am encouraged to seek licensed medical professionals for any medical or psychological concerns.

RELEASE OF LIABILITY

I hereby release, waive, and discharge Esthetics by Monica, its owner, practitioners, employees, and affiliates from any and all claims, demands, damages, or legal actions arising from or related to services rendered, except in cases of gross negligence.

POLICIES & FINANCIAL AGREEMENT

I agree to comply with all posted business policies, including but not limited to:

  • Cancellation and rescheduling policies

  • Payment and refund policies

  • Membership or package terms (if applicable)

CONSENT & ACKNOWLEDGMENT

I certify that I have read and fully understand this agreement. I have had the opportunity to ask questions, and all of my questions have been answered to my satisfaction.

By booking an appointment , I acknowledge that I voluntarily consent to all services rendered and agree to the terms outlined above.

PRODUCT RETURN & REFUND POLICY

At Esthetics by Monica, all retail product purchases are made with care and professional recommendation. Due to the nature of skincare and wellness products, we maintain the following policy:

ALL SALES ARE FINAL

For hygiene, safety, and quality control reasons, opened or used products are not eligible for return, exchange, or refund under any circumstances.

UNOPENED PRODUCTS

  • Unopened, unused products in original packaging may be exchanged only (not refunded) within 7 days of purchase

  • A valid receipt or proof of purchase is required

  • Products must be in original, resalable condition

ALLERGIC REACTIONS / SENSITIVITY

We understand that skin sensitivities can occur. If you experience a reaction:

  • Please notify us within 48 hours

  • We may offer a product exchange or store credit at management discretion

  • No cash or card refunds will be issued for reactions

Patch testing and proper use as instructed are the client’s responsibility.

DISCONTINUED / SALE ITEMS

  • Products purchased during promotions, sales, or clearance events are final sale

  • No returns, exchanges, or credits apply

DAMAGED OR DEFECTIVE PRODUCTS

If a product is received damaged or defective:

  • Notify us within 48 hours of purchase

  • Product must be returned for inspection

  • Replacement or store credit may be issued if approved

ONLINE ORDERS (IF APPLICABLE)

  • Shipping fees are non-refundable

  • Esthetics by Monica is not responsible for lost, stolen, or delayed packages once shipped

NO REFUNDS ON SERVICES

All services, packages, memberships, and gift cards are non-refundable.

FINAL ACKNOWLEDGMENT

By purchasing products from Esthetics by Monica, you acknowledge and agree to this return and refund policy.
